I understand that, but even despite such entry there was a failure. 
Interestingly, the error report includes a space within the quoted string:

        home phone error with placeholder [< patientcomm ::′ homephone′ >]

despite that what I put in the text template had no space

        & home phone $<patient_comm::'homephone'>$\\

so does that maybe point to the problem?
