The first was from Rogerio's referral template, using the list type 
"description" which seems best suited to where there are two components 
returned.

The second was from the template medslist.tex (Current medication list (GNUmed 
default)).

Did you want me to generate both a PDF and a pdflatex log of the second, and 
post them?
