At the top of every source file put
Foobar, Copyright 2007 Penguin. See the file "LICENSE" for license
information. Nothing else grants any rights.
In "LICENSE" put

GNU General Public License with the exceptions and restrictions listed
below. A copy of the GNU General Public license is in the file GPL. If
