I try to locate this bug, but not completely successful. I find this 
only occur
when using TrueType fonts with iso10646 registry through X with 
FT_OPTIMIZE_MEMORY
enable. By using xfd, I find, with freetype has FT_OPTIMIZE_MEMORY 
enable, the width,
left (LeftSideBearing ?) and right (RightSideBearing) value are not 
correctly computed.
But I'm not quite understand freetype library, it a little difficult for 
me to find
the exact bug.
