[fluid-dev] More CVS checkins

Another set of CVS checkins for those who would like to or are willing
to test it :)

Changes include:
- More leaks plugged (thanks go to Paul Miller for the patch)
- Print a warning when a command line argument SoundFont/MIDI file is
not found (thanks to Nick Daly for pointing this out)
- Reference documentation in the process of being improved
- Some optimizations with audio drivers when user audio callback not
being used
- Other code cleanups

Stephen Grant:  I have not yet gotten around to looking at the issues
you mentioned for the Windows build.  Will do that at some point before
next release.  But thanks go to you also for noting issues with the
Windows build.
