I solved this problem by reinstalling the system fonts. I used a program called Pacifist to unpack the fonts from the Tiger installation disc and restored them to /System/Library/Fonts and / Library/Fonts. Although the Monaco font worked fine in other programs, it was unreadable by Emacs for some reason.
I can reproduce the problem by removing Monaco.dfont from /System/ Library and rebooting.
