I think I have seen this too some time ago (GNU/Linux redhat 9 on x86).
IIRC, it typically happened in Gnus.
But I never seen any specific pattern...
Since I disabled the blinking cursor some time ago (it obstructs
debugging of redisplay errors), I haven't seen any disappearing
cursors... and had forgotten about it.
