Follow-up Comment #1:

Thank you for delving into this stuff. I have done all the work related to
getting titles and title pages and so on working to the extent that they do
work, but I have never really understood what LilyPond is doing. Fairly
recently I created the titling system that you will find under
Score->Titles->BookTitles and Movement->Titles->BookTitles.
These scripts allow you to create a title page, table of contents and a
critical commentary (via Score->Titles->BookTitles->Epilog) gathering up the
Directives->Critical Commnent directives and numbering them placing them at
the end, with any notes created via the Title... command in Epilog). An
example of this is http://www.denemo.org/dl29 which is created without any
LilyPond tweaking. 

The Simple Titles scripts still work, they involve some horrible kludge (IIRC
the Title for the work is created as a title= entry, and then each movement
title is created via subtitle= entries).

Just now going into the repository is further development allowing you to
quote music snippets in the commentary, and already delivered is an incipit=
field that places a snippet of music on the title page so that you know what
the music is from looking at the title page. This is enough to allow the
creation of thematic indices, where the table of contents gives the incipit of
each movement. But I am struggling here with my lack of understanding of the
\markup command - at the moment comments use \wordwrap-string but once you
insert a snippet of music the word-wrapping fails, so its use is limited to
short comments about the music.

It would be nice if some hero stepped up to create an ideal flexible system
for this stuff, but I can see that it is a lot of work. The Book Titles work
that I have done is built on the book-titling.ily file of Nicholas Sceaux, and
he clearly put a lot of work into it, but I couldn't live with the output -
the automatic upper-casing jarred on my sensibilities and so I overrode some
of the code by including the file simplified-book-titling.ily and it is in
there that I have now included printing the incipit field on the title page.

However, even without designing a complete system, if you create a menu of
commands to create the titles as you would like them they can be placed in the
menu system for others to use - that too would be most welcome.
