[Denemo-devel] MIDI in on startup

I have added a pref to start the MIDI in on program start, but excluded
its action from _HAVE_JACK_ as I am not sure whether it is relevant or
how it would work in that case - if it is irrelevant then it should not
be displayed in the dialog for the JACK build - it will show at present.
If it is useful for JACK then please adjust the code.
