ddd on MacOSX

Dear All,

There's a nasty gotcha for people trying to compile ddd 3.3.1 on MacOSX:
there's no distinction between filenames that differ only by case. This
means that the userinfo executable overwrites the USERINFO text file and the
ddd executable overwrites the Ddd application defaults file. Judiciously
changing these names by hand and altering the Makefile will fix this but
it's pretty messy.

It then falls over on the "-i system" argument to gcc which is easily fixed
by changing to "-I" and the namespace part of cxxtest needs to be removed.

Then it all compiles nicely - whew!

Cheers
Bill
