There is a package auto-pst-pdf (which I tried out a bit, but it is not
further developped) and the package pstool (which you mentioned and
which is actively developped). As far as I understand both cut out
automatically the diagramm, make LaTeX and DVIPS with it, psfrag and in
the end there is a ready made PDF diagramm with the psfrag alterations
included -- and this now will be included into the main text again. 

All that works automatically by the package. But it means, that under
the hood you still need  good old LaTeX. Because you precompile your
diagramms with that before you compile everything with PDFLaTeX.

When trying that, I had great difficulties with the input stack size, I
don't have under LaTeX. When I realized, that I don't get completely
rid of LaTeX that way, I stopped my experiments...
