Follow-up Comment #2, bug #37569 (project grub):

Confirm to have hit this issue while setting up a lvm over raid1 setup with
debian-installer 7.0 rc1: grub 1.99, os-prober 1.56. Installation was
successful but received the same unknown LVM metadata header errors when
running update-grub (part of a kernel package upgrade).

After manually upgrading to grub 2.00-13 and running updade-grub I received
these warnings:

# update-grub
Generating grub.cfg ...
/usr/sbin/grub-probe: warning: Couldn't find physical volume `(null)'. Some
modules may be missing from core image..
Found linux image: /boot/vmlinuz-3.8.0
Found initrd image: /boot/initrd.img-3.8.0
/usr/sbin/grub-probe: warning: Couldn't find physical volume `(null)'. Some
modules may be missing from core image..
Found linux image: /boot/vmlinuz-3.2.0-4-amd64
Found initrd image: /boot/initrd.img-3.2.0-4-amd64
done

Which left the box unbootable.

I suspect any fresh installation with d-i 7.0 rc1 consisting of lvm over raid
with grub 1.99 will reproduce this error. Hope this helps.
