grodvi currently renders dollar sign ($) always in upright roman font variant.
This is because in Knut's italic cmbi10 and cmti10 fonts used by TBI and TI
are missing dollar sign glyph. But there are slanted Knut's fonts cmbxsl10 and
cmsl10 which have dollar sign. Therefore grodvi for TBI and TI should use
dollar sign from cmbxsl10 resp. cmsl10 font. cmsl10 is not real italic only
slanted variant, but still it is better then upright roman variant.
