I've given my opinion in my reply to Juri:

  Act on each file when any non-empty part
  of its line is in the active region.

So far, the actions we're talking about are
only _marking_ actions (mark with * or some
other mark, flag with D, or unmark).

IMO there's no need for any kind of error,
warning, or confirmation requirement when
just (un)marking.

And for commands that _use_ marks, acting on
the marked or flagged files, we already do
require confirmation when it makes sense.
