ldap-host-parameters-alist customization duplicates item

Please describe exactly what actions triggered the bug
and the precise symptoms of the bug:

In lisp/net/ldap.el, the checklist in the customize definition for
ldap-host-parameters-alist contains the "Search Base" tag (and
associated information) twice.  One of those should be removed.

I suggest the first one be kept and the second one be removed, since
this is a _very_ common customization and so deserves to be near the
top.
