problem with rtag command missing files

This is the first time I have used this list, so please pardon me if I am
doing the wrong thing. Please assume stupidy not malice.

The problem I am seeing is that very infrequently the rtag command seems to
miss a file when I tag (either a tag or a branch tag) a module in my
repository.
These doesn't seem to be a pattern to the file be missed (both text and
binary files have been missed). 

Has anyone see anything like this behavior, or are they any suggestions as
to how to diagnose the problem further.
