CVS has an internal list of file names it ignores because they are
things you typically don't want in your repository (object files,
executables, backup files, etc.).  If you want to import absolutely
everything, add a -I! option to your import command.

No; CVS manages files, not directories.  You should have your build
process create them as needed.
