Look in axiom sources : src/doc/book.pamphlet is the latex source of the
book. It can be compiled with latex ou pdflatex without any issues
(except graphics for pdflatex).

The correct command is:
\usepackage{hyperref}

Because you haven't used pdflatex command. ;)

Unfortunatly without graphics. I just ran pdflatex on the book.pamphlet
file.
