I've now opted for a different scheme.  One can define complete
alternatives with a "||" separator.

As an example, the default value for \item in `TeX-fold-macro-spec-list'
now is "[1]:||*".  This means, in case an optional argument is present,
display its value followed by a colon, i.e. "\item[foo]" becomes "foo:".
In case no optional argument is found, use the alternative string, which
is "*".

The code is available in CVS if somebody wants to test it.
