My best debugging efforts (read adding printf("here!\n");fflush(stdout);) has led me to line 160 in lib/pdf/pdf.cc:
    delete i->outputDev;i->outputDev=0;

It seems that the last time I see my printf say here! is right before this line.

My test pdf is located at http://www.irs.gov/pub/irs-pdf/fw4.pdf (first pdf result in google for 'pdf')

Can't reproduce this. Would you happen to have a gdb backtrace?
