Why do you need this in addition to the GEN_VXFORM_DUAL() invocations above?

GEN_VXFORM_DUAL() sets invalid bits to zero whereas these new instructions
have different values. So I have defined GEN_VXFORM_DUAL1() to pass the invalid 
bits.

The opcode of vinsertd is not shared with any other instruction and it has a 
different
invalid bit other than zero. So I have used GEN_VXFORM_300_EXT() to handle it.
