I rebased it on master.

The outstanding issue is how we treat FUA on non-writing commands.
I sent a message to lkml to determine how it treats it. I have my
views (as per the message) but I thought I'd wait until lkml
had its say as that was the interface we were originally trying
to emulate. I think we are going to end up with it means
nothing, server SHOULD NOT sent and client MUST ignore.
