sorry, the "Confirm Opt Out" link, which is missing the form action as rendered - this does it for me. If I request HEAD for that Link, I get the "A fatal error has occurred..." etc. message. I am jumping to system lynx out of pine; I will see if I get it on other systems.

I just tried that (going to that link, giving 'v' to all cookies), and
don't see a problem.  It just gives me this page (perhaps some more
information would help):
