Index doesn't appear fully

I have and index at the end of my PhD.  It's not big, just 2 pages but for
some strange reason the second page is not generated.  I get:

unresolved cross reference @Runner&&following
    16,1: too little horizontal space for galley @IndexSection at @Galley
lout file "Doctoraat.ld":
  1028,8: galley @IndexSection deleted from here (no target)
...

And it doesn't go away.  I've noticed this problem appearing and
disappearing as I was writing my Ph.D. but sadly, now I'm finished, the
problem is back and I haven't figured out how to get it to go away.
I don't see why it is claiming that there is no space for the galley since
it is at the end of the document and take as much space as it wants.

Any help would be welcome,
