For midi I don't know, but for pdf or png you can use LilyPond itself.
When you \include "lilypond-book-preamble.ly" in your file, lilypond
will create individual output files for each system.
If you can print one bar per system (use manual breaks and "ragged-right
= ##t" in the paper block) you'll have individual files per measure.
