Hi. This is a quite good question : there's a Stem_engraver, and it seems that Note_heads_engraver includes noteheads + all other engraver dealing withi the note... any engraver big boos around to explain ? While wainting for an accurate explanation, I give you a trick to do what you want : \override NoteHead #'transparent = ##t ... the NoteHead context deals with the note head...
