Is it just the music examples that are offset or the full document?
In the latter case, it must be a general problem with your TeX
installation, nothing specific for LilyPond.
Try for example the commands
latex sample2e
dvips sample2e
which should produce sample2e.dvi and sample2.ps with a 3 page document.
The easiest way to get A3 layout is to use the "geometry" package in
LaTeX. Run the command
textdoc geometry
to see the documentation.
