I could add a comment to the code with a TODO for this - there was no consensus for how to fix it, but it is a bug and at least I've found what's causing it.  Hopefully the comment will spare someone the time of trying to find cause of the bug later on.  Lemme know if this sounds OK.

This is partially but not totally fixed.  This will not truly be able to be marked as fixed until LilyPond can do a form of optimization that allows her to figure out stem direction and horizontal spacing concurrently.  It should stay.

Still working on it (it's one of those problems that would require a major code rewrite for a not-much-better visual output in very rare cases, so it is waayyy low on my priority list but I don't want to forget about it).

Still working on it.  There is still no consensus as to how to represent tremolos in horizontal spacing: not having them in the column leads to collisions in tight spacing, whereas having them in the column leads to too-wide spacing.
