`make dvi' problems

I know.  Part of them are due to special formatting for the
manual. The majority of the warnings are caused by the ancient
notation engine - a part which doesn't follow coding conventions so
strictly.

The manual is made with internal-type-checking set to #t, so I am
confident that there are no real problems.
