I append an extract from a Brahms Waltz, set for accordion and published by Edition Schott in 1933. This is from a time where accordion notation was not all that well-established and/or standardized, so it leans back on piano traditions.

Standard basses for accordion have only 12 different single bass notes (that can be executed using different registers) and assemble chords from only 12 different additional notes (again, multiplied by using different registers).

Today's instruments feature combination registers where single combination register keys establish a combination of reeds. At the time this piece has been set, it was more customary to have single controls switching reeds individually on and off. Switching a single reed could be done using the thumb without apparent disturbance visual and acoustic disturbance of play.

That explains that the instructions here call for playing several interspersed bass notes in a lower octave. Note that the actual play button being pressed will remain the same: it is the registration that needs to be switched to the lower octave.

At any rate, the notation chosen is a single italic 8 below the bass note. Both the numbers as well as the italics distinguish the octavation from a fingering (seen elsewhere).

So at least for rendering this piece in LilyPond accurately in both visuals and Midi, being able to shrink an octavation mark to a single centered italic 8 would be called for.
