Add the following two lines to your $HOME/.ssh/config file:

Host jimrandomh.org
    User address@hidden

Now you should be able to use

    export CVS_RSH=ssh
    cvs -d :ext:jimrandomh.org:/cvs init

with no problems. The ssh executable itself will send the correct
identification for you.
