You can force the direction of a fermata or any other script by
using '^' or '_', for example: d4^\fermata

You can move the text further away from the notes/staffs by adding
some extra padding:
\property Staff.TextScript \override #'padding = #2
similarly, you can add padding to several other kind of objects.
