Remember when you changed to gnuhealth user to install the server?
su - gnuhealth

Now it is saying to go back to your normal user(not root necessarily, your default GNU/linux user), you can go back with the command exit or just open a new terminal and keep that terminal with the logged gnuhealth JUST to run the server.

The installation guide is a wiki, http://en.wikibooks.org/wiki/GNU_Health/Installation , remember that you can improve it with more clear explanations, I bet Luis and the other devs are pretty busy with the code itself.
