We were exchanging thoughts with Chris and Andrew about improving the usability of the patient evaluation process and there, we talked about the reporting of the clinical history. 

We'll be also making changes on the signs and symptoms. Yesterday, we added the Present Illness  in the main form.

It seems i am not following the right path

1) I added a new report record from Administration/Actions/Reports
2) As the report didn't show up on Patient  (tryton client view)
3) i added two entries in health_report.xml
4) I went for upgrade option for health module in order for changes to take effect.
5) The new report option showed up on Patient (tryton client view) but ...

Question:

 Now it is showing 2 entries of the same type in  Administration/Actions/Reports ?  Did i do something wrong ?

I tried to mimic entries of other reports. I didn't understand why there is a Keyword "Print Form" in all three reports Medications, Vaccinations and Diseases and model value is Pediatric Symptoms Checklist Evaluation. Is it of any use ? 

The best way is to declare them in the XML files. Take a look at the current reports in the "health" module. That should give you some tips on how to declare the report and their links.

Take a look a the current default branch, where we're making the changes for the next stable release. We are working heavily on this section since yesterday. I think that the main / basic clinical history report should be within the "health" module. Although a more specialized reporting would need a different module .
