The linegap and vertical advance are not related, you should not try
to add them.. !!

The text height can be computed as "ascender+descender+linegap".

If believe that if you take the values from the horizontal header,
you'll compute the baseline-to-baseline distance for horizontal
layouts.

I assume that the values from the vertical header correspond to
the baseline-to-baseline distance for vertical layouts, even
though it's a bit unclear from the spec..

You can't use both the advance height and the text height to
advance the vertical pen position, it's either one or the other..
