I resurrected my virtualbox disk containing cygwin (gcc 4.3.4) and could
install pastiche and run its tests suite and the example application. I
cannot reproduce the sendfile issue.

I run salmonella on some eggs.  Everything went as expected, as far as I
can see.  I get the same results as salmonella on tests.call-cc.org.
Here are the outputs:
