To get the promotion popup you should make sure that "(Almost) always promote to Queen" is not ticked. When it is ticked you, must select other pieces by first dragging the 7th-rank Pawn backwards until you get the desired piece. This is why it says "Pull Pawn backwards to under-promote".

In setup mode the piece is selected by the amount of (vertical) movement between the button press and release; you just move until you see the piece you want, and release then. It should also be possible to step through the piece type by repeatedly right-clicking the same square.

This method of setting up a position is only recommended for late end-games, with just a hand full of pieces. For complex position it is much more convenient to click the active clock until the 'palette board' appears, where each piece type occurs exactly once. You can then drag the pieces where you want them (or off board when you don't need them), using Ctrl to duplicate what you need twice (and add Pawns by simple right-clicks).

I have Xboard 4.9.1 running on my MacBook Pro with High Sierra. When I am in the Edit Position mode, the right mouse click does not work. In the older Windows version, I could clear a board, then right click on any square. That would bring up a menu of pieces I could place on that square. On my MacBook, any time I right click on a square, a White Pawn inserted there (e.g., see screen shot below).
