I'm not sure this is a bug, since piano accidental styles are set at
GrandStaff/PianoStaff level. For the default style to work properly
temporarily, it also needs to be set for PianoStaff.
This works fine:

#(set-accidental-style 'piano-cautionary)

#(set-accidental-style 'default 'PianoStaff)

#(set-accidental-style 'piano-cautionary)
