Issue 312: minimum-Y-extent is ignored in vertical spacing

The point is that there should be large spaces between the systems -- 20 units 
above the top staff and twenty units below the bottom one. I've attached the 
output 
I get and I think it's correct. The old behaviour was that the systems would be 
placed next to each other at the top of the page.
