I recommend to use the syntax c4:16 for single note tremolos, see
http://lilypond.org/doc/v1.8/Documentation/user/out-www/lilypond/Tremolo-subdivisions.html
When I try your example in the latest development version of LilyPond,
I actually notice a bug, namely that all notes following the
\repeat "tremolo" 4 command get a tremolo, not only the first one.
Example:
\repeat "tremolo" 4 b16 c4 d2
Incidentally, I notice that there are frequent language errors in the
English-language version of the manual. Unless you're planning a major
revision of the manual in the near future, I would be happy to proofread
the current version and submit a list of these errors.
Please do! Even better is if you can correct the source files and
send patches.
