[Bug-gnulib] getline.h and unicodeio.h self-contained

Hi,

I made this trivial change. It makes getline.h and unicodeio.h self-contained.

        * getline.h: Include <stddef.h>, for size_t.

        * unicodeio.h: Include <stddef.h>, for size_t.
        * unicodeio.c: Don't include <stddef.h>.
