The typical way to do what you want is to use one
level of indirection...

echo <<EOF > $HOME/bin/ssh-p-4242
#! /bin/sh
exec ssh -p 4242 ${1+"$@"}
EOF
chmod +x $HOME/bin/ssh-p-4242
CVS_RSH=$HOME/bin/ssh-p-4242; export CVS_RSH
cvs -d :ext:address@hidden:/usr/cvs version
