1) What version of Axiom are you using?

2) How did you install Axiom?

3) How did you install TeXmacs? From source? From binary?
   Which binary version?

4) The tm_axiom interface program that is distributed with
TeXmacs should be configured to run only the 'AXIOMsys'
program - **not** the 'axiom' shell script. This is the
standard configuration. Some people modify tm_axiom to
also run the Axiom graphics, but this requires some
additional changes to tm_axiom.c and re-compilation.

5) When you type 'AXIOMsys' in the console, you should see
only the main Axiom process start, with only the banner
information and one '(1) ->' prompt. If you see more than
one prompt, then you probably have an 'AXIOMsys' script
that starts Axiom in the wrong mode. The symptoms of
this would be similar to what you describe because the
extra prompt breaks the communication between Axiom and
   TeXmacs.

Let me know if this helps. Or if you have more questions.
